اعلان حول اصدار جوازات طبعة (A)

تهدي سفارة جمهورية العراق في كندا اطيب تحياتها الى الجالية العراقية الكريمة ويسرها ان تعلن قرب اصدار جوازات السفر طبعة ( A ) لذى يرجى من المواطنين الكرام الراغبين باصدار الجواز تقديم المستمسكات الاتية  :

 1- إملاء نموذج الاستمارة باللغتين (العربية والانكليزية) أو اللغتين (الكردية والانكليزية ) ، من الموقع الرسمي لمديرية الجوازات العامة   http://www.iraqinationality.gov.iq/eForm/.

2. هوية الأحوال المدنية الاصلية .

3- شهادة الجنسية العراقية الاصلية .

4- في حالة عدم وجود لقب في هوية الاحوال المدنية يتم تثبيت اسم الجد الثالث بدلاً من اللقب .

5- بطاقة اقامة دائمية أواجازة السياقة الكندية او اية وثيقة ثبوتية كندية اخرى .

6- صورة فوتوغرافية حديثة جدا عدد 4 ذات خلفية بيضاء وبقياس (45 × 35) ملم .

7- حضور ولي الامر مع مستمسكاته الثبوتية لمن هم دون عمر الستة عشر عاما لغرض اخذ البصمة  الحية  لكليهما.

8- تعتمد شهادة الجنسية العراقية للأب بالنسبة لمن هم دون سنة الثانية العشرة لمن لم يحصل  منهم على شهادة الجنسية العراقية.

9- لايجوز اصدار جواز سفر من هذه الطبعة لمن هو حامل لجواز سفر طبعة (G) النافذة.

10- عشرون دولار امريكي نقدا او صكا مصدقا.

 ملاحظة هامة :

 لغرض تنظيم العمل في السفارة وبغية التأكد من الاوراق الثبوتية قبل مراجعة المواطن، يرجى ارسال صور ملونة (Scanner) مـن ) هوية الاحوال المدنية، وشهادة الجنسية، واستمارة الجواز المملوءة بالبيانـات والصـور الفوتوغرافية( مع رقم الهاتف، وايميل المراسلة على ايميــل السفــارة passport@iqemb.ca او ارسال نسخ ملونة بالبريد العادي لغرض تدقيقها ومن ثم سيتم الاتصال بالمواطن على رقم هاتفه او ايميله لمنحه تسلسلا وتاريخا للمراجعة.

Announcement about Issuing Passports Type (A)

The Embassy of the Republic of Iraq in Canada presents its compliment to the dignified Iraqi community and it is pleased to announce the near issuance of passports type (A); Iraqi citizens are required to provide the following documents to apply for passports type (A):

1.              Filling out an application form in (Arabic and English) language or in (Kurdish and English) language, available at the official electronic website of the Passport General Directorate:

      http://www.iraqinationality.gov.iq/eForm/

2.              Original civil status identification.

3.              Original certificate of the Iraqi nationality.

4.               In case the civil status identification does not hold the Family name then the third name will be replaced as the last name instead.

5.              Residence card or Canadian driver’s licence card or any other Canadian document.

6.              Four recent photographs with a white background and measurements of (45x35) mm.

7.              The guardian of a child under 16 sixteen years old must also be presented in person with the above mentioned identification documents in order to provide live fingerprint for both.

8.              The certificate of the Iraqi nationality of a father will be approved for his minor children who are under 12 twelve years old and who have not obtained an Iraqi certificate of nationality.

9.              You may not apply for obtaining a passport of this type if you hold a valid passport type (G).

10.         $20 USD in cash or certified check.

Note:

To organize the work at the Embassy and to verify the identification documents, A citizen is advised to send a coloured copy scanning the following documents before arriving to the Embassy: civil status identification, certificate of the Iraqi nationality, filled passport application form, photographs, phone number and email address to this email: passport@iqemb.ca, or by the regular mail. A citizen will be contacted by a phone or by an email provided by her/him on the form to be given a sequence number and a specific date
